Program areas at Healthcare Educational and Research Fund
Herf was awarded a grant, funded by the nys health foundation, the samuels foundation and the health foundation for western and central new york. The purpose of the project is to work with the health systems to become age-friendly, improving care for patients, families, caregivers, and Healthcare providers. The first contract ended october 2021, at which point new york community trust joined for the second year of the contract. The second contract ended december 2023, with the exception of samuels that was extended to december 2024. Phase iii of the grant will begin in january 2024 and will include all four partners.
Herf was awarded a grant funded by mother cabrini health foundation. The purpose of the project is to work with the health systems to address health disparities by advancing Healthcare equity and inclusion through education and training. Herf provided hospitals and health systems with education, data analysis, and tailored community-based interventions to address disparities. Education, resources, and support were provided to those who work directly with the community to improve outcomes for all patients, but especially underserved and disadvantaged new yorkers.
Herf planned and managed a wide variety of Educational programs, seminars, and briefings for the hospital and nursing home members of hanys. Topics covered included managed care, behavioral health, the interpretation and strategic use of Healthcare data and information, leadership and supervision, quality improvement, continuing care, emergency preparedness, new york state and federal Healthcare policy, and health information technology.
