Program areas at Healthcore Clinic
Healthcore Clinic provides access to comprehensive family medical services with a focus on education, prevention, and early intervention. The integrated model of care incorporates dental, behavioral health, and social determinants of health screenings and brief interventions that are coordinated during the medical exam process. This integrated model reduces stigma, improves access to behavioral health and dental services, and promotes team-based patient-centered health outcomes.
Healthcore Clinic provides culturally appropriate mental health and substance abuse services designed to reduce stigma associated with seeking behavioral health support. Most patients are screened for behavioral health concerns during the medical appointment. Working collaboratively with the medical and dental team, behavioral health consultants provide coordinated and integrated care to patients and families with concerns ranging from stress management, anxiety, depression, trauma, and behavioral support for other mental health and medical diagnoses.
The dental Clinic has seven (7) chairs and diagnostic equipment to provide comprehensive dental services including exams, cleanings, sealants, oral cancer screenings, teeth whitening, root canals, dentures, extractions, crowns, and walk-in emergency services. Most medical patients are screened and provided oral health education during routine medical appointments.
