Program areas at Herkimer County Health Network
Rural Health Network programs:hchn's activities under its new york state department of Health rural Health Network funding supports the Health and well-being of individuals who live, work, play and learn in Herkimer County and the mohawk valley by addressing the following priority areas: access to quality healthcare, chronic disease prevention and self-management, healthy communities, obesity prevention, access to healthy food, physical activity and nutrition. Highlighted activities under these priority areas include: sponsors medical transportation programs, child dental Health education, establishment of mohawk valley mile walking trails, increased access to vegetable/healthy foods in community markets, disease self-management classes and nutrition seminars.
Other programs performed by hchn include; sponsoring the national diabetes prevention program, diabetes self- management program, chronic disease self-management program, sponsoring an intergenerational community center project which is a grant from the community foundation of Herkimer and oneida County, lead screening programs known as the excellus lead level screening grant and convening workgroups to address identified Health priorities.
The senior initiative age-friendly: go local funding will support initiatives that are built on the key principles of age-friendliness, livability, and equity to improve the lives and Health of residents in their communities. Herkimer County healthnet is working with the age friendly Herkimer County coalition, director of office for the aging, and other members of the community to develop a workplan for our age friendly process in Herkimer County.
