Hospice Care delivering Hospice & Palliative Care to the residents of warren, essex, franklin, hamilton,st lawrence and Washington counties - a 5,400 square mile, mostly rural service area the agency provides 24/7 clinical Care, teaching and support to individuals and families livingwith life-limiting illness, the agency provided Care based on individual Care plans developed for each patient and family an interdisciplinary team of nurses, social workers, chaplains, aides, volunteers and others provide the expertise, treatment, medication, equipment and supplies needed to support the best quality of life possible for the individual and family bereavement support and counseling is provided to Hospice families and community members who are living with loss the program works to address the causes of physical, emotional, spiritual and social distress that interfere with quality of life the agency is licensed by nys and is medicare/medicaid certified
