Program areas at Community for Positive Aging

Senior centerthe Senior Center is the physical location and the heart of cfpa. It's where older adults come together to learn, attend events, volunteer, and receive support and services.during the year ending june 30, 2023, the multipurpose Senior Center provided more than 2,536 in-person and zoom events and classes with 24,632 participants.
Community health navigation, vaccinations and other Oregon health authority covid 19 responseto address disparities that low-income people of color face in utilizing and navigating health resources and systems, cfpas multilingual Community health team provides education, outreach, and one-to-one coaching around resources and services including emergency preparedness that help people live long, healthy, and meaningful lives. Last year, the Community health team connected with more than 1,600 Community members. Additionally, cfpa hosts vaccination clinics to make the covid vaccines and boosters more accessible to the low-income seniors and older adults in subsidized housing. With cares act-covid 19 oha funding ending, cfpa is seeking new contracts and grants to continue this important work.the staff and board of directors continue to closely monitor developments that may impact either revenue sources or the ability to deliver programming and will continue to make adjustments accordingly.
Aging, disability and veteran servicescfpa operates as the lead agency of the n/ne consortium in partnership with the urban league of portland. Cfpa contracts with the multnomah county Aging, disability and veterans services division to provide assessment and care coordination for older adults living at home in northeast/north portland who need assistance to maintain their independence. Cfpa works with more than 350 clients to help older adults remain in their homes as long as possible.during the year ending june 30, 2023, cfpa provided 2,358 care coordination hours to clients and their caregivers. Services include a comprehensive care coordination assessment, arrangement for needed services including housekeeping and bathing assistance, respite for caregivers, day care, home delivered meals and transportation, and follow-up and re-evaluations. Cfpa responded to 1,596 requests for information and assistance from other organizations, Community elders and their family caregivers.transportation assistance services focus on improving access to critical services to include providing monthly bus passes, tickets, coordination for door-to-door transportation for medical appointments, grocery shopping, and activities in the Community. Cfpa provided transportation assistance and coordination to 271 seniors improving their access to vital services and programs in the Community.
