Program areas at Hope Cooperative
Case management: our comprehensive case management services support adults with psychiatric disabilities, housing insecurity, and other challenges. Services include benefit acquisition, crisis intervention, housing assistance, employment support, transportation, therapy, medication management (in specific programs), group activities, and life skills development. Individuals are also linked to primary care, legal aid, financial literacy training, and social connections to foster meaningful community engagement. More than 3,200 individuals received case management services.
Homeless outreach program: the homeless outreach program (hop), in partnership with the city of sacramento's department of community response, conducts street outreach in encampments, focusing on connecting individuals to shelter and essential services. The team provides limited case management, street medicine referrals, assistance with ids, and registration into the homeless management information system (hmis) to access housing resources. Annually, the hop team connects with over 12,000 individuals.outreach and engagement center: the outreach and engagement center (oec), in collaboration with the city of sacramento, offers temporary shelter for individuals experiencing homelessness. Guests receive short-term case management, housing assistance, and referrals to mental health, substance use treatment, primary healthcare, and veterinary services. The oec also accommodates pets, ensuring individuals and families receive support together. With a nightly capacity for 50 guests, the program serves singles, couples, and families, providing vital stabilization services during times of crisis. Alcohol and drug treatment services: Hope Cooperative offers tailored outpatient and intensive outpatient alcohol and drug treatment programs. Clients receive individualized support through weekly one-on-one counseling and group sessions designed to help them achieve recovery goals. The program annually serves more than 200 individuals successfully in making progress in their recovery journey.
Residential services: Hope Cooperative's residential services provide housing solutions for individuals experiencing homelessness, including those with mental health challenges, substance use disorders, and transition age youth. Types of housing include temporary shelter, transitional housing, and permanent housing, with onsite supportive services such as life skills training, financial literacy, crisis intervention, and tenancy-sustaining assistance. Individuals also receive linkage to mental health, substance use treatment, employment services, and volunteer opportunities. 950 individuals received residential supportive services.
Crisis respite services: the crisis respite center provides a safe, supportive environment for adults experiencing a mental health crisis who are not a danger to themselves or others. Staff offer compassionate support to help clients de-escalate, stabilize, and develop an action plan to prevent future crises. The center responds to over 3,500 crises annually, ensuring individuals receive immediate, solution-focused care in a welcoming setting.
