Hospice of Marion County provides comfort centered care to residents of Marion County with advancing illness (a prognosis of 6 months or less). We coordinate an extensive plan of care overseen by an interdisciplinary team of medical professionals, including physicians, nurses, certified nursing assistants, social workers, chaplains, pharmacists, bereavement facilitators and trained volunteers, who address the patient's physical, emotional, psychological, social and spiritual needs. The Hospice team guides patients and their families through the difficulties of dealing with a life-limiting illness by providing compassionate care in one's own home, in one of our three Hospice houses, assisted living facilities, nursing homes or anywhere a patient calls home.
Hospice of Marion County offers numerous opportunities to enhance community education, grief support and caregiver resources. When a family is overwhelmed by the news of advancing illness, our staff is standing by to assist with the necessary information and connections to help others make informed decisions that are best for their family. Our ancillary programs include a pre-hospice program, transitions, nancy renyhart center for dementia education to train the care givers of those suffering with dementia, telemonitoring units in the home, faith-based trainings, living with chronic disease workshops, grief seminars and bereavement camp for youngsters aged 5-12 and teens all supported by generous donations thus available at no charge to the public. Our goal is to enlighten the public by demystifying the role of Hospice care to encourage earlier referrals so that every patient in Marion County may live life to the fullest, despite the advancement of serious illness.

Ccpc was the only palliative consulting company in the nation recognized for excellence and as a model for others by the national consensus project for its expert management of pain and symptoms for patients at any stage in their illness in the hospital setting; that care has extended to facilities and some home visits. The coordinated relationships between our companies ensure an exceptional range of services which often reduces costs and offers consistency of care. Each company is a distinct llc within its area of expertise, staff and management.
