Infinity Health (ih) was funded as a federally qualified Health center (fqhc) in decatur county, Iowa, in 2005. Today, ih manages eleven locations, two of which provide primary and preventive Health care services. The region served is incredibly rural, and accessibility can often present a barrier to Health care. The communities served are in great need of the accessible, affordable primary and preventive Health care, and the organization is committed to ensuring that the communities' needs are met. The organization currently serves approximately 10,000 unique patients, conducting approximately 52,000 patient visits annually (2023 uds). Of these, approximately 17,400 visits are for medical services. The organization currently has 5.5 fte medical.
Infinity Health provides oral Health services in the leon, osceola, and mt ayr, Iowa facilities. The organization currently serves approximately 10,000 unique patients, conducting approximately 52,000 patient visits annually (2023 uds). Of these, approximately 6,700 visits are for dental services. The organization currently has 4 fte dental providers.
Infinity Health (ih) is the only fqhc in the state of Iowa that has also received the designation of being a community mental Health center (cmhc). Ih currently provides mental Health and substance abuse services in the Iowa counties of appanoose, lucas, monroe, wayne, ringgold and decatur. Telemedicine services are provided in each of the seven locations. The organization currently serves approximately 10,000 unique patients, conducting approximately 52,000 patient visits annually (2022 uds). Of these, approximately 22,300 visits are for behavioral Health services and 1,900 are for substance abuse services. The organization currently has 18.5 fte behavioral providers and 2 fte substance abuse providers.
Infinity Health began providing pharmacy services in 2017. The pharmacy fills approximately 37,000 scripts yearly and currently has 3 pharmacists.
Infinity Health opened a behavioral Health access center in december 2019 which provides crisis stabilization services. The services that are provided are out of scope of the hrsa grant. Access center services were ended 8/31/2023
Infinity Health (ih) is also licensed to provide substance abuse services. Ih currently provides substance abuse services in the Iowa counties of appanoose, lucas, monroe, marion, wayne, clarke, and decatur. The organization currently serves approximately 10,000 unique patients, conducting approximately 52,000 patient visits annually (2023 uds). Of these, approximately 1,900 are for substance abuse services. The organization currently has 2 fte substance abuse providers.
