I.SOCIAL DETERMINANTS OF HEALTH:2018 Outputs include: provided leadership and staff support for a regional collective impact cradle to career initiative called Southern Oregon Success which has a mission to weave together community expertise and resources to promote the health, well-being, academic and life success of children and families; activities included a half-day community learning summit focused on various major initiatives in our region related to children and families with over 100 participants from multiple sectors; the Southern Oregon Success initiative developed a community dashboard to track key metrics including, but not limited to, high school graduation rates, 3rd grade reading proficiency, and kindergarten readiness.II.REGIONAL COMMUNITY HEALTH ASSESSMENT:2018 Outputs include: convening and coordinating a Regional Steering Committee with representatives from 16 partner organizations to establish a collaborative regional assessment process to create greater alignment of organizational community health improvement plans and ensure best use of community resources; activities included development and implementation of a community engagement strategy, convened monthly meetings of Steering Committee as well as small work group meetings, provided administrative support and served as local coordinator for yearlong process, served as communications hub for the this two-county assessment process, applied for and managed reporting of community grants/funds, managed partner cost sharing agreement and project budget, responsible for all reporting to board and community.III.JRHA ADMINISTRATION:2018 Outputs include: launched redesigned website; held quarterly board meetings and monthly executive committee meetings to address mission and goals and nurture the key relationships of this learning community of leaders; created quarterly reports for board including project updates; conducted annual election of board and executive committee; submitted reports to funders as required; served as neutral convener for myriad small group and partner meetings; managed day to day operations.
