Outpatient integrated behavioral Health services - Jamhi provides outpatient mental Health services to adult, children, and adolescent members of the community. These services include integrated behavioral Health assessments with "same day" walk-in service; psychiatric evaluations, primary care services, individual and group therapy, case management, rehabilitation services, assertive community treatment services, intensive outpatient program (iop) and medication management (including medication assisted treatment). Jamhi works closely with families, community partners, primary care providers, and others to provide integrated behavioral Health services. Clinical, case-management, psychiatric, and primary care professionals work in partnership with persons served to develop individualized treatment plans aimed at recovery. Jamhi offers tele-behavioral Health and telemedicine services.
Psychiatric emergency services - Jamhi provides 24/7 on-call response with immediate screening and crisis intervention for adults experiencing mental Health emergencies. These services are short-term, intensive psychiatric services provided to individuals during an acute mental Health crisis episode. They include clinical assessment to determine the need for psychiatric hospitalization and coordination of services for support and referral. Psychiatric and primary care/nursing services - psychiatric services provided at the Jamhi clinic, by a psychiatric advanced nurse practitioner and supported by nursing staff, include psychiatric evaluation, medication management, and case consultation. Nursing staff also oversee medications provided to those residing in Jamhi residential programs. Clients are provided education and assistance with accessing financial supports to help cover medication costs. Medical and support staff provide primary care medical services on-site at the Jamhi midtown clinic. Active coordination between psychiatric and primary care staff within Jamhi, and with providers and other resources in the community ensures that medical and psychiatric Wellness is an integrated process for persons served. Jamhi psychiatric and nursing staff are committed to the "whole person" approach to psychiatric and primary care with a focus on Wellness. Jamhi also offers stand-alone primary care services to individuals in the community on a sliding fee scale. Case management/rehabilitation services - Jamhi has a team of case managers who provide community supports for seriously mentally ill adults and seriously emotionally disturbed children and adolescents through outreach services. Case management also provides training in life skills that encourage independent living. Coordination and integration of services throughout the community and among other agencies is a key element of service. Persons are assisted with applying for eligible benefits and programs, finding and maintaining housing, and accessing resources to help them live as independently as possible in the community. Jamhi provides support to family members as well as clients.
Housing - safe, affordable housing is essential to improved quality of life and recovery for persons with serious mental illness and co-occurring disorders. Jamhi's continuum of housing for adults offers opportunities for persons who might otherwise be homeless due to the significant challenges they face in finding and maintaining a place to live.
