Program areas at Lakeside House

Community residence there were 14 distinct individuals served during 2023. 5 individuals were discharged after completing their goals and 4 were admitted. Occupancy was 96% for 2023. Lakeside House community residence looks forward to omh approval to make necessary changes to its physical structure to meet ada compliance. We are currently accepting bids to do ada compliant upgrades to the cr building and were approved by omh to begin the construction phase.
Care management services health home is a care management service that connects community and social supports with health care and provides better organization of medical and behavioral health care. Health home ensures all caregivers involved with a person talk to one another so they can focus on their client's needs. During 2023, we served 21 clients. We are listed with adirondack health institute and hired our first full time employee to assist with the program in 2023.
Community services another's purpose a vocational program designed to assist residents with opportunities for employment as well as volunteer activities in the community. Another's purpose also provides indoor and outdoor activities such as field trips to the wild center in tupper lake, shopping in malone and plattsburgh. Also, activities such as movies, bingo, wii bowling, camping, bowling, badminton, bocce ball, cornhole, etc. We also run a pantry store to assist with building a business-like atmosphere allowing residents to take responsibility for running a store, selling, buying, cashing out, keeping track of stock, etc. Another's purpose served 28 individuals during 2023. Community support services program helps individuals in the community who are not formally connected with other services but who may have emergency needs such as clothing, food, transportation and at times financial assistance. During 2023, we provided assistance to 14 individuals. The supportive housing program continued providing ongoing assistance to 13 individuals through 2023. This program is designed to assist individuals to maintain housing. Services include rental assistance, as needed, and visits at least one time per week to assist with paying bills, grocery shopping, medication management, and transportation to doctor appointments.
Transitional housing services samaritan House is an 8-bed home, serving homeless people primarily from franklin and essex counties. Samaritan House provides services to assist people with social security benefits, banking, job employment and works with agencies such as essex co. dss, franklin co. dss, ecumenical council, salvation army, and other religious denomination affiliations. Samaritan House recently received temporary certification under the guidance of otda. We look forward to permanent certification in 2024. We also hired a new program manager. We served 65 clients and had an 88% occupancy rate.
