Supported by funding from the margaret a. cargill philanthropies (macp) and the ucare Foundation, the Foundation continued its work on the connected communities for healthy aging project. Two rural pilots were selected to create integrated care systems across aging services, healthcare, and community supports. Assistance was provided to plan, implement, and evaluate outcomes. Objectives include: convening and advancing local and collaborative whole system, whole person approaches to serving seniors in community; experimenting and improving care of medicare advantage enrollees in a local health plan in coordination with providers of primary care, acute- and post-acute care, home care, and community-based services; connecting consumers to local resources for healthier aging; implementing effective workforce strategies to support this collaborative model.
The Foundation operates an asynchronous online nursing assistant training program. This program addresses the delays and barriers in accessing cna training, increases the quality of the curriculum and training, and meets all testing requirements.
The Foundation offers scholarships to help staff and caregivers from our member organizations advance their professional development. Recipients are selected through a review process and approved by the board of directors. In 2022 scholarships totaling $33,600 were awarded.
Workforce: the Foundation provided workforce grants to many senior living communities to support them in recruiting and retaining directcare workforce employees.
