Crisis and as support supports individuals and community members and first responders with 24/7 phone and onsite support to meet the needs of the community. These crisis support services are provided through outpatient, inpatient, or virtually as circumstances warrant.life Management Center's emergency services professionals are on call 24 hours a day, 7 days a week to provide crisis counseling or conduct emergency mental health evaluations that include suicide assessment and examination for inpatient psychiatric hospitalization.crisis counselors help individuals examine their problems and evaluate possible solutions or treatment alternatives. Counselors assist in making referrals or transfers to Life Management Center services or other appropriate service agencies and community resources.appropriate auxiliary aids and services will be provided to qualified persons with disabilities at no cost to the customer or companion, to ensure all customers/companions have equal access to and an opportunity to benefit from services.
Florida assertive community treatment (fact)fact is an evidence-based program that provides intensive, community-based treatment, rehabilitation, and support services for adults with severe and persistent mental illness who have not responded well to traditional treatment. Many of those served by fact have histories of repeated hospitalizations, admissions to state hospitals, multiple emergency room visits, homelessness or incarceration in local jails. Services are provided in a location convenient for the client, including within homes, work sites, jails, hospitals, and community settings.the goal of the program is to lessen or eliminate the debilitating symptoms of mental illness experienced by individuals in order for them to become productive members of our community.
Community action team (cat)the cat program provides intense behavioral health care services to youth where traditional interventions have been unsuccessful. The team provides family-centered services which are individualized according to the strengths and needs of each youth and family. The overall goal is to support and sustain youth within the family system, in the community and in school. Social service supports are integrated with behavioral, trauma and substance abuse interventions. Medical staff provides psychiatric care and basic health status checks, and works with the family to maintain medical records and linkages to community health care practitioners. The whole family is embraced in care and family commitment and participation is essential and expected.the program is open to youth ages 11 to 21 who meet two or more of the following criteria:are at risk for out-of-home placement;have had repeated psychiatric hospitalizations or outpatient treatment failures;have been involved with the department of juvenile justice or multiple episodes involving law enforcement;poor academic performance, multiple suspensions or school drop-outs".
Life Management Center provides comprehensive, integrated behavioral and mental health care in bay, gulf, jackson, Washington, holmes, calhoun counties by promoting the health and quality of Life of our community members. Services are provided to individuals, families, and groups in a variety of settings regardless of a patient's ability to pay.
