Lmvna provides the highest quality care in the most cost-effective manner to the residents of ventura county. Home health and palliative patients receive skilled nursing and therapy (pt, ot, st) as prescribed by their physician. Our clinicians bring all of the same services of a clinic directly to home bound patients. Qualifying low-income patients receive assistance from our social workers to apply for the medi-cal program to help offset the cost of their treatment. Patients with inadequate insurance or no insurance who meet income qualifications can have the cost of their care waived. We provide service to any patient, regardless of their ability to pay for care. During the past year our clinicians provided 29,516 visits to 2,619 patients.
Lmvna offers a wide range of grief and support services through our grief and bereavement program. All services are offered free of charge to any ventura county resident. Services include one-on-one counseling, support groups and in-school groups for teens. Licensed mft and msw counselors work with mft associates to provide appropriate grief services to individuals, couples and families. During the past year 704 adults, children and teens received services through this program. Our staff conducted 1,506 individual counseling sessions; 388 individuals attended group counseling sessions; and 12 schools partnered with lmvna to provide services to students.
Hospice is a comprehensive medical model and provides the full range of hospice services to patients who meet the criteria of a limited life expectancy. We specialize in the care of people with advanced illnesses - most often for patients in the last six months of life when a cure is no longer an option. Our team-oriented care considers the whole person and their family - focusing on the quality of life through pain management, emotional support, and spiritual care. During the past year, we provided care to 488 patients for 36,268 days on care.
