Menorah Campus Health Services

Menorah Campus Health Services, Inc. was formed for the sole purpose of establishing and operating a multi-specialty diagnostic and treatment center within the meaning of article 28 of new york public Health law and the new york codes of rules and regulations adopted pursuant thereto. Menorah Campus, Inc., the sole member of Menorah Campus Health Services, Inc., is also a member of fallon Health weinberg, Inc. ("fhw"), which operates a managed long term care program ("mltcp"), a dual eligible special needs plan ("d-snp"), and a program of all inclusive care for the elderly ("pace") that will serve primarily low income frail elderly residents of western new york. A diagnostic and treatment center is required by new york state regulations in order to operate a pace program and as a result, the primary activity of Menorah Campus Health Services, Inc. is providing Services to pace program participants. Menorah Campus Health Services, Inc. Provides diagnostic and treatment center Services, primarily medical care, podiatry, physical therapy, occupational therapy, speech language pathology, psychological, dental and radiology Services. Menorah Campus Health Services, Inc. Provides Services to a largely medically underserved low-income senior and elderly population.
