Program research and evaluationadvancing Health literacy - university of California, san diegomhf established an initiative titled - heart and soul Health connection to provide interventions to hypertensive participants to help decrease blood pressure and achieve better cardiovascular outcomes among african american/blacks. The county epidemiology data, 3/19/22 lists the number one common underlying condition of covid-19 associated deaths is hypertension at 42%. Cardiovascular disease-related mortality is the leading cause of death in the united states, with hypertension being the most prevalent and potent risk factor. Hypertension has disproportionately affected african americans who have poorer outcomes and a higher burden of associated comorbidities. The covid-19 crisis was particularly devastating among african americans who demonstrate increased incidence and poorer Health outcomes related to the disease (nih). The goal of this project is to provide interventions to include nutrition education, exercise and community Health worker engagement to participants to assist them in managing their hypertension while continuing to reach unvaccinated residents with covid education to build trust and increase vaccination rates. Augmented by a grant $23,000 west Health institute
Community support grants and awardsmhf worked with multiple organizations to help provide outreach and engagement support. Grants $ 5,000. Richard Health & associates/california department of public Health, vaccinate all 58 - community covid outreach. Mhf conducted covid vaccine education, outreach, and engagement activities to address equitable access to the covid-19 vaccine and information on its safety, and continued building trust among african americans/black residents.
Covid community outreach and engagement servicesgrant $77,387. Mhf worked with sdwc to reach managed care members in areas that had low vaccination rates to conduct covid vaccine education and engagement activities to continue building trust and increase the vaccination rate among underserved residents. Although vaccination rates have increased, they are increasing at a slow rate in african american/black residents due to a historical lack of trust that lingers in the black communities. Mhf worked with medical entities to hold community vaccination clinics at target locations in targeted zip codes to gain greater access and have a greater reach. Focused on blue shield promise and from Health net members.
