Founded in 2012, MyClinic is an independent, nonprofit 501c3 organization with the following Mission: MyClinics team, including volunteers and collaborative partners, is committed to empowering individuals and families who have limited resources to improve their health and quality of life by providing information, access to clinical care and community services. Over the past year, MyClinic has shifted its focus from a medical clinic, that offered primary care and certain specialty services, to a Health and Wellness Navigation Hub already serving over 3,500 registered clients and committed to expanding our services and network throughout northern Palm Beach County. Current services include Care Coordination and Navigation for medical and social services, Womens Health (Promise Fund), Behavioral Health Counseling and Nutrition and Wellness programs as well as facilitating on-site appointments with outside resources including SNAP and other benefits.
