Program areas at Nixon Home Care
Program 1: Nixon Home Care, Inc. Provides transportation services for otherwise home-bound low-income elderly or impaired individuals. Cdc identified gaps in opportunities for seniors and disabled adults to regularly engage in physical activity. They cited this as probable causes of serious mental illness and the development of chronic conditions later in life. Cdc research also found that social isolation is a significant problem for impaired adults and that they often sedentary lifestyles contribute to feelings of depression and anxiety. These emotions are linked to obesity and a number of other chronic illnesses including cardiovascular diseases including diabetes. But, one of the biggest challenges elderly and impaired adults face is finding reliable transportation to take them to and from needing services and to and from recreational and social activities. Cdc estimates that the annual health Care costs of caring for people with obesity-related disabilities is approximately 44 billion annually. In this fiscal year, Nixon Home Care, Inc. Provided over 4,000 units of transportation to over 150 low-income elderly and impaired individuals and covered over 160,000 miles providing these services within 12 months. Our seven-van fleet provides clients with door-to-door round trip transportation to and from our centers between 7 a.m. and 5 p.m. weekly and to and from social activities and appointments. We serve the entire 1,729 square mile harris county, Texas region and all drivers are certified and licensed to ensure the safety of our clients during transport regardless of their level of disability.
Program 3: Nixon Home Care, Inc. Provides nutritious dietician-approved congregate meals and snacks for low-income elderly and impaired adults. Chronic hunger is a persistent and ever present issue among seniors and impaired low-income adults. The texans feeding texans home-delivered meal grant program was established to help supplement and extend the applicant's current home-delivered meal program for established to help supplement and extend the applicant's current home-delivered meal program for annually since 1996. In this fiscal year, we provided over 80,000 congregate meals to over 150 low-income seniors and impaired adults. The daily meals included one hot noon meal that includes two ounces of meat, 1/2 cup of fruit/vegetables, one cupof milk and two servings of bread and mid-morning and mid-afternoon snacks, served between 11 a.m. and 1 p.m. with feeding assistance (i.e. Spoon feeding, bread buttering and milk opening) when necessary. In addition, we serve mid-morning and mid-afternoon snacks between 11 a.m. and 1 p.m. all meals are prepared in our facilities and served according to federal and state nutrition and food handling guidelines and in accordance with doctor-ordered dietary restrictions. This is our way of ensuring that the clients we serve receive the proper levels of food intake to help them maintain a balanced diet and avoid the clients we serve receive the proper levels of food intake to help them maintain a balanced diet and avoid.
Program 2: Nixon Home Care, Inc. Provides health services, assessments, maintenance and personal Care within a safe caring environment. Harris county seniors prefer to age in place and continue their community relationships. However, in order to honor their wishes to remain living independently in their homes for as long as possible, the impaired adults must have access to health and personal Care services designed to ensure that they maximize every opportunity to live healthy and productive lives. In this fiscal year, Nixon Home Care, inc.'s licensed nurse and trained personal attendants provided over 150 low-income seniors and impaired adults with over 40,000 units of health services that included: (1) stabilization of clients' health conditions, treatment and medication management; (2) monitored health assessments and counseling to advise client/caretaker of the client's immediate and long-term health needs; (3) personal Care services, including assistance with dressing, eating, grooming, bathing, toileting, and self- administering medication; and (4) physical rehabilitative services including motion exercises. Last year, we also established an on-site 5-workstation computer lab also empowers our clients to help us monitor their physical health and chronic health conditions.
