Program areas at Opengate Incorporated
Intermediate care facility (icf): provides housing and day services to 34 individuals, with a focus on assisting those that are dually diagnosed. Opengate employs a person centered approach utilizing personal outcome measures for all individuals in our residential care. Opengate provides active treatment programming and clinical services to individuals in this program and works to ameliorate behavioral challenges.
Day services: comprised of day habilitation, community habilitation and supportive employment programday habilitation : Opengate operates a large day habilitation program that offers both site-based services and services in the community referred to as our day hab "wow" program (without walls). Individuals enrolled in this program focus on their valued outcomes and enjoy many enriching activities within the community. Activities include volunteer opportunities focused on acquiring job skills, recreational activities such as bowling, movies, miracle league baseball, visits to the microsoft store for x-box games as well as site-based recreational and learning activities.community habilitation: community habilitation offers participants and their families the flexibility of having services delivered by Opengate support staff in a variety of community settings. Community habilitation is goal oriented and offers support and skill development in the areas of daily living skills, travel, health and wellness, adult education, communication, socialization, leisure skills, money management, positive behavioral support, safety skills, self-advocacy, informed choice, community integration, and relationship building.supportive employment program: supported employment is a person-centered employment planning and support service that provides assistance for an individual to obtain, maintain and advance in a job. Opengate has a dedicated team of professionals that support in the areas of career exploration, resume building, application assistance, interview preparation, employment training, interfacing with employers and advocacy.
Individualized residential alternatives (ira): ira programs provide residential services to 66 adults with idd. These homes provide enriching opportunities for growth through the person centered planning process. Valued outcomes are developed for each individual and residential habilitation outcomes assist individuals with meeting their goals.
