Family support services - funding provided by nys opwdd. This funding is used To provide information & referral, training and Parent matching services To parents and other family caregivers of children (adult and minor) with intellectual or developmental disabilities or children suspected of having the same. Services are also provided To professionals who provide supports and services To these families and/or their children.
Special education core community partner - To provide information, referral and training regarding special education and transition planning for families and professionals of students birth - 26 years of age. The program seeks To promote the meaningful involvement of families within their child's educational program. Parent To Parent of nys is responsible To provide these services in a xx county region surrounding the capital district.
Family To family health information center - funding provided through the federal maternal child health bureau, a division of hrsa, To provide information & referral, training and support To Parent and other family caregivers of children, birth - 21, with special health care needs. Special health care needs, as defined by mchb, includes children with developmental, physical, emotional and behavioral health care concerns. Working with State agencies and professional providers To improve systems of care for children with special health care needs and To promote the informed participation of families within systems development is also part of this work.
