Park Center, Inc. is a community mental health Center (cmhc) with offices in allen, adams, wells, huntington, and whitley counties. Our goal is to provide quality care and support to promote wellness and healing for clients of all ages and at every stage of recovery. At Park Center, Inc., we offer a comprehensive selection of services in the professional setting you expect, while tailoring treatments to achieve the personal care you deserve.in 2023, Park Center, Inc. Expanded intensive outpatient (iop) programming. An additional 6 iops were added to Park Center's programming in 2023, bringing the total number of iops offered to 18. (see schedule o for continuation)specialized iops to serve the community in a robust manner were added to programming, including women's trauma, anxiety and obsessive-compulsive disorder, adolescent substance use, and though/psychotic disorders. This resulted in over 4,000 additional iop billed days in 2023. During 2023, Park Center, Inc. Continued improvement work for access to outpatient services by continuing to streamline the assessment model. Park Center, Inc. also has walk-in access monday through friday for clients to receive crisis care without waiting and walk-in wednesdays for urgent medication follow-up appointments for existing patients. Additionally, Park Center continues to focus on development of therapy groups for clients regarding mental health, trauma, substance abuse, and dialectical behavior therapy (dbt). Another accomplishment in 2023 was the continued expansion of the mobile crisis team (sos) to address a wide variety of crises including suicide. Our team of licensed social workers, mental health counselors, marriage and family therapists, addiction counselors, psychiatrists, physician assistants, advanced nurse practitioners, and psychologists are uniquely qualified to address behavioral health needs. Park Center, Inc. offers specialized services in early childhood mental health, dialectal behavioral therapy (dbt), medication assisted treatment (mat) for opioid use disorder, eating disorders, and intensive outpatient (iop). Services are available to individuals across the lifespan. Park Center, Inc. offers community-based services in allen, adams, huntington, whitley and wells counties. These services are available based upon an assessment of need. Our team of therapists and caseworkers work alongside individuals in their home, school and community environments.park Center, Inc. has a 16-bed acute psychiatric hospital for adults located in allen county. This facility provides short-term stabilization, assessment, medication review and treatment planning. Prior to discharge, clients are assisted with creating plans for safety management and linking to other less intensive services.park Center, Inc. has a number of adult group homes in which we serve clients with mental health and substance use needs. Park Center, Inc. also has a residential addictions program for individuals, including those referred by the court system.park Center, Inc. utilizes a hospital liaison and a court liaison to assist consumers with higher needs. The hospital liaison provides support services to individuals at state or local psychiatric hospitals. The court liaison collaborates with consumers, families and the court system when the severity of symptoms requires emergency detention, involuntary commitments and guardianship.financial policy:park Center, Inc. is committed to making healthcare accessible to those who need it, regardless of their health status or ability to pay. With our financial assistance, patients can receive the parkview care they expect - no matter what medical obstacles they might face. Our financial assistance services include: 1) using fair and consistent billing and collection practices, including charitable care and financial assistance policies and a standard application process; 2) providing extended payment plan options; and 3) providing emergency care, regardless of ability to pay.community benefit: Park Center, inc.'s commitment to improving health and inspiring well-being extends beyond the hospital walls and throughout northeast Indiana. Through parkview's community health improvement (chi) program, Park Center, Inc. collaborates with others inside and outside the health system who share parkview's mission and values. Funded by a percentage of parkview's net operating surplus, chi strategically reinvests dollars into community organizations and initiatives that address the greatest health needs outlined by the hospitals most recent triennial community health needs assessment (chna). Based on the 2022 chna results, the top three health concerns for Park Center, Inc. are mood disorders, childhood trauma and substance use/abuse. Based upon these results, Park Center, Inc. Creates an implementation strategy that serves as an in-depth plan defining how we plan to address each identified health need.
