Program areas at Partner for Mental Health

Partner for Mental Health (pmh) provides community navigation for residents of charlottesville, albemarle, and the central Virginia area. Pmh's clients tend to have serious Mental illness and/or substance use disorder, often have also experienced significant trauma, and are some of the most vulnerable members of our community. for many, their illness has been poorly managed and has led to estrangement from their families and their communities. Pmh works to mitigate interactions with law enforcement by working with the albemarle district court's therapeutic docket and the albemarle charlottesville regional jail's re-entry program and to prevent legal entanglements by partnering with albemarle county's co-responder program, hart, to do follow-on calls to persons calling 911 with Mental Health or substance use crises. Pmh works to connect clients with Mental and physical Health providers and social service supports (including housing, employment, food, income supports, etc.) To facilitate them living stably in the community. In april 2023, pmh celebrated five years of providing community navigation for the therapeutic docket clients. The docket has graduated more than 130 clients during that time. Pmh also, entered an mou with the charlottesville albemarle recue squad to provide follow-on services to 911 callers, and began negotiating a contract with albemarle county's co-responder (hart) program as well. Pmh's intensity and duration of client support varies with each client's profile of needs but includes educating, advocating and connecting them to a range of Mental and physical Health providers, housing, transportation, employment, financial supports, and other community resources. More than 80% of our clients make significatn progress toward their recovery goals.
