Program areas at Partners in Care - Oahu Continuum of Care
Homeless management information system ("hmis"): Partners in Care ("pic") is the hmis lead acting as the fiscal agent for pic. The pic coc data committee provides over to hims operations. The pic coc advisory board provides policy guidance through the data committee for hmis operations. Migration of coordinated entry into hmis, which includes working with our database vendor to not only customize but make system-wide changes to meet the needs of ces and all the work they do to facilitate getting people into housing. Our team has successfully completed all required federal reporting in record time with improved data quality checks and updated our system to meet the most recent hud data standards. We have maintained public-facing dashboards showing data quality / completeness and inflow-outflow numbers. Hmis had supported the point-in-time / housing inventory count operations, which included cleaning data for hud submission and publishing public sub-reports (community report, domestic violence report, veteran report, native Hawaiian report, kupuna (seniors) report).
Coordinated entry system ("ces"): the ces team has spent the bulk of this year meeting with hud ta from icf, researchers from university of hawai'Hawaii'i manoa, and our dedicated workgroup of providers leading this work forward. The workgroup went through a process of evaluating the vispdat. Uh researchers did a racial analysis of vispdat questions to identify gaps and barriers of native hawaiians and other overrepresented racial groups. From there, the workgroup collected extensive feedback from providers of all perspectives (outreach, shelter, housing programs, healthcare representatives, etc.) To identify topics, questions and processes that are missing from the vispdat/our current assessment process. The ces team reviewed the feedback and compared it to several other off-the-shelf assessment tools until we landed on one that met the majority of the provider's wishes. If the tool identified is approved by the Oahu coc committees and advisory board, it will become our new ce assessment tool. Ongoing monitoring will be necessary to ensure that the tool is proven to be equitable but making it to the voting phase has been a major accomplishment for the Oahu coc.partners in Care Oahu has also been partnering with community solutions' built for zero program over the last year. Locally, we have developed the Oahu veterans initiative to end homelessness to work towards functional zero in collaboration with several community Partners and stakeholders. A large part of this process has been refining veteran data to ensure we are telling an accurate story and capturing all self-identifying veterans. While the focus is currently on veterans, our intention is to carry out similar work plans with other subpopulations in the future.
Coc planning: Partners in Care ("pic") is a coalition of representatives of organizations from nonprofit homeless providers, government stakeholders, private businesses, community advocates, public housing agencies, hospitals, universities, affordable housing developers, law enforcement, and persons experiencing homelessness and formerly homeless persons. Pic is a planning, coordinating, and advocacy alliance that develops recommendations for programs and services to fill needs within Oahu's Continuum of Care ("coc") for homeless persons. Pic assists in developing new programs, while working to preserve or expand effective existing programs. As a member of Hawaii statewide coc, pic works collaboratively with the rural counties' coc, bridging the gap. Within the year, significant strides were made under the hud planning grant. One of the key accomplishments was the comprehensive revamp of the monitoring and evaluation ("m&e") process, aimed at enhancing accountability and the effectiveness of program implementation. This restructuring ensured that performance tracking was more robust and aligned with strategic goals.the team conducted a pivotal strategic planning session, during which ten priorities were established to guide future initiatives and decision-making. This session laid a strong foundation for targeted, strategic actions. in addition, a specialized strategic planning session was held specifically for people with lived experience (""le""), addressing their unique challenges and ensuring their perspectives were integrated into the broader planning framework.to further support preparedness and enhance grant application processes, five preparatory meetings for notices of funding opportunities ("nofos") were developed and facilitated. These sessions equipped stakeholders with the knowledge and tools needed to navigate funding opportunities effectively, fostering a proactive approach to securing resources.together, these accomplishments strengthened the foundation for sustainable progress and positioned the hud planning grant initiative for continued success in the coming years.
