Care coordination: the organization completed 2,502 holistic comprehensive assessments for older adults, their families and caregivers during fy23. Based on the identified needs of the older adults, organization staff made the appropriate referrals to state and federally subsidized programming to assist the older adult to age in place safely in the community for as long as possible.
Adult protective Services: during fy23 the organization investigated 532 reports of abuse, neglect or exploitation of an older adult or an adult aged 18-59 years old with a disability. Case workers conduct investigations and work with in resolving the abuse, neglect, or financial exploitation situation
Title iii-c2: home delivered meals are provided to individuals over 60 years of age who are either temporarily or permanently unable to manage meal preparation and access food. The nutritious home delivered meal service doubles as a wellbeing check. This is critical for individuals experiencing social isolation. During fy23, the organization delivered a combined 53,205 meals, hot and frozen. Additionally, the organization coordinated delivery of 14,604 special diet meals and 2,933 shelf stable meals.
