Description
Patient Care Intervention Center improves quality and reduces costs by coordinating care and integrating data for vulnerable populations in the health care system. The current model of care is siloed, which forces individuals who seek care from multiple agencies to have a case manager represent them at each provider. However, these case managers lack the technology needed to effectively manage patient needs.

Program areas at Patient Care Intervention Center

The current model of care for vulnerable populations is siloed across and within medical and social providers. Individuals who seek care from multiple agencies have a case manager to represent them at each provider, but these case managers lack the technology to collaboratively coordinate care across systems. As such, referrals between systems are passive, placing the responsibility of coordinating visits upon the most at-risk individuals. To solve these issues Patient Care Intervention Center has developed the Unified Care Continuum Platform, which bolsters population health at many levels to ensure accountability and coordination across provider systems. PCIC has secured data from 850+ agencies to integrate medical, social and behavioral health through a comprehensive platform approach. Our platform brings together over 46 million health records representing more than 6.6 million unique individuals in Harris County and surrounding counties. Our referral platform has generated over 20,000 community referrals to help our community partner agencies help their vulnerable clients and communities.
