Hospice house: provides patients and their families a resource for inpatient skilled care at the end of life in their own community. Hospice provides not only the medical care that a patient requires, but also care for the emotional, social, and spiritual needs of the patient and their loved ones. Under the direction of a physician, the following teams tend to all of the needs of the terminally ill: registered nurses, certified nursing assistants, social workers, chaplain, and other medical personnel. As part of the standards of medical, emotional, and spiritual support service provided by Pocono Vna-Hospice this team acts on a plan of care that is an individually tailored and well-choreographed approach with the goal of easing the stress and pain at the end of life. Patients served totaled 245 with 1,061 days in care in fiscal year 2023.

With hospice, patients can remain at home - alert and pain-free - relaxed, comfortable, and most important, surrounded by their loved ones. We have a special team of trained professionals and volunteers to provide physician directed skilled nursing, home health aides, therapy, dietary, social work services, and pastoral care. This team acts on a plan of care that is an individually tailored and well-choreographed approach with the goal of easing the stress and pain at the end of life. Hospice patients served totaled 375 with 10,862 days in care in fiscal year 2023.
