Premier Health group (phg) is a non-equity model physician hospital organization created to clinically integrate physicians and hospitals in the common interest of achieving the triple aim: improved consumer experience with the healthcare system and lowering total cost of care, while maintaining or improving quality outcomes. Phg serves as the provider network for and provides continuous quality improvement functions for Premier Health plan, Premier Health insuring corporation and Premier Health aco of Ohio, all affiliates of Premier Health. Through our physician-led organization, we deploy various population Health models to our providers to help them better coordinate care for their patients in our efforts to serve Premier Health's mission of building healthier communities.
