Adult care facilities and services for the accommodation of adults with physical or other limitations who are not acutely ill and not in need of hospital care where no adequate housing exists for such people. For the year, program accomplishments are as follows: 1. 44 individuals received home care services; 2. Provided 18 beds for adult care services; and 3. Provided 78 beds for assisted living services.4. 156 individuals received health home case management services;
