Health resources and services administration (hrsa) projects-innovative solutions is the development of a rural mental health network which consists of partners representing mental health, behavioral health, primary care and social services collaborating to provide clinical and community-based services through integrated area coordination. Innovative strategies is the continuation of the foundational work of the innovative solutions project designed to implement a collaborative care team model utilizing practitioners and clinical providers to serve as team members to collaboratively address the clinical and social health needs of high risk clients. Impact is a rural Communities opioid response program implementation project that addresses prevention, treatment and recovery needs of clients through a continuum of care provided through a coordinated recovery oriented system of care. Caring communities/youth empowerment zone (como youth works) - Caring Communities serves the community facilitator in creating linkages between community partners, key stakeholders and residents to address emerging community issues to enhance the quality of life for all residents. Areas of emphasis are upon mental health, child emotional development, positive youth development, womens health and responsible fatherhood. Youth empowerment zone mmp/youth development specialist provides services through the Missouri mentoring partnership program in the areas of employment, education, health lifestyles.
Missouri foundation for health projects-convening community for conversation is a project designed to stimulate health-focused discussions in the west end community with the goal of mobilizing the necessary stakeholders, skills and resources to form a coalition that will address health issues from a local policy perspective with broad community participation. The accountable health community project consists of the design, develpment and delivery of an accountable health community model that will utilize care coordination services and systems nagvigation through the incorporation of community health workers. The accountable health addresses a critical gap between clinical care and community services in the current health care delivery system. Everybody counts is a census 2020 outreach, awareness, education and enrollment for Randolph and howard counties. University of Missouri curators - stronger together Missouri project provides the national parent leadership training institute to train leaders for civic engagement. The project also provides the parent cafe training institute and parent cafe to train parents and community leaders in implementation of parent cafes designed to strengthen families through the incorporation of five protective factors and opportunity to share expertise and experiences in a healthy and safe environment.
Missouri department of health and senior services - usda summer food provides nutritious meals for youth the age of 18 & under free of charge during the summer months when school is out of session. Community health worker class recruits individuals to attend the community health worker certification program and to increase the engagement of community health workers (chws) to promote linkage between health systems and community resources for adults with high blood pressure, diabetes, and high blood cholesterol.

Randolph County united way-magic city express provides low cost affordable public transportation services for individuals to a diversity of locations within the community.

Innovative strategies support the improvement and creation of behavioral health systems that will better coordinate and deliver services, maximize resources and pursue policies that promote an integrated network of partners to facilitate positive change.
Childrens trust fund - investing in our families, a family nurturing program that incorporates the strengthening families framework and five protective factors model as the common theme throughout the project. The project targets high-risk families that have children ages 0-9 and have greater potential for committing child abuse and neglect due to factors such as lack of parenting skills, lack of an adequate support system, and ongoing life crisis.
