Mi choice medicaid waiver program: provides personal care, community living supports, adult day care, chore, private duty nursing, training, homemaking, environmental modification, respite care, home delivered meals, transportation, counseling, personal emergency response systems, durable medical equipment, self-determination, and other services.
Title iiic-1 and iiic-2 nutrition programs: congregate and home delivered meals, including nsip, to seniors aged 60 and older.
Title iii-b administration, title viia and elder abuse prevention: provides transportation, outreach, personal care, case coordination and support, chore, legal assistance, home repair, assistance to the deaf, long-term care ombudsman, senior center staffing, program development, disease prevention/health promotion, health screening, senior center operations, information and assistance, care transitions, medication management, assistive devices, and elder abuse prevention for seniors aged 60 and older.
Scsep, state funded programs, veterans directed, mmap - provides training resources, covid19 vaccinations, social isolation mitigation resources, social isolation outreach opportunities, nursing facility transitions services, va supports coordination and related services, and medicare/medicaid guidance and information.
