Program areas at San Juan Basin Area Agency on Aging Incorporated

Home services provide assistance to the elderly with daily in-home activities. The Aging and Disability Resource Center (ADRC) program provides a coordinated and streamlined access point to long-term services and support for adults age 60 and over, or age 18 and over living with a disability, and their caregivers. ADRC empowers older adults and adults with disabilities, and caregivers, to navigate health and long-term care options by providing counseling in a person-centered manner.
Nutrition services provide basic services and activities intended to provide older persons with assistance in maintaining a well-balanced diet, including congregate and home-delivered meals, nutrition counseling and education.
Transportation and assisted transportation are available under the Older Americans Act (OAA) to enable older persons to travel to and from congregate meal sites, community facilities, and other resources for the purposes of applying for or receiving services, reducing isolation, and promoting independent living.
