Program areas at Sangre de Cristo Community Care

Hospice and Palliative care provides a more dignified and comfortable experience to terminal patients and their families. Our team of professionals and bereavement counselors provide an integrated, holistic approach to healthcare, no matter where that person may reside. Sangre de Cristo Hospice and Palliative Care has been the preferred choice of terminal individuals and health care professionals in the area for over 30 years. Quality is reflected in our CMS Compare 4 Star rating.
The Sangre de Cristo Joni Fair Hospice House located in Pueblo is the only one of its kind in southern Colorado. The inpatient unit provides acute symptom management to return a patient to their home or long term care environment. Our care focuses on the comfort of patients and their families.
Our Home Health program provides services in southeastern Colorado rural areas where there are limited healthcare resources. We provide compassionate clinical care and multiple therapy disciplines in order to help each patient reach their goals and have successful rehabilitation outcomes.
