Program areas at Sangre de Cristo Hospice and Palliative Care

For many seriously ill patients, Hospice and Palliative Care offers a more dignified and comfortable alternative to spending one's final months in the hospital. Sangre de Cristo Hospice and Palliative Care (sdchpc) has been the preferred choice of terminally ill individuals and health Care professionals in the area for over 30 years. Medicare's featured tool Hospice compare compares Hospice centers based on the quality of Care they provide. Sdchpc rates are higher than all other regional hospices and higher than the national averages.
Our inpatient unit located in pueblo is the only one of its kind in southern Colorado. It is staffed with professionally trained teams of physicians, nurses and certified assistants, social workers, chaplains, and volunteers who help provide quality, compassionate Care in a home-like setting. The goal at Hospice house is to establish an environment of skilled Care for both patient and family members.
Our home health program covers most of our rural a clinical and therapies, we serve those in need of outcomes have been successful and this program appears to have growth potential.
