Assertive community treatment program produces intensive supportive crisis community based treatment 24 hours a day, 365 days per year to the chronically mentally ill population in jackson and hillsdale counties. The variety of services increased stabilization and safety to the population served.
Other services include family psychoeducation, outpatient therapy, etc. These help consumers and their families find solutions, build relationships and develop recovery based skills. Outpatient therapy includes activities designed for the purpose of reducing maladaptive behaviors, maximizing self-control, restoring normalized psychological functioning, reality orientation and/or emotional regulation and re-motivation thus enabling improved functions and more appropriate interpersonal and social relationships. This includes psychotherapy, brief solution-focused therapy, and dbt skills therapy.
Dreams provides case management services for the severe and persistent mentally ill in jackson and hillsdale counties in Michigan. The services include care coordination, support, and linkage to community resources to the less intensive mentally ill population.
Medication delivery is offered to consumers who have chronic difficulties taking their medications as prescribed which could result in hospital or crisis home admissions or may prevent the need for more restrictive services. The intent of the program is to assist consumers in identifying and implementing strategies to take their behavioral health medications as prescribed with the goal of moving the person from daily medication drops to taking their medication independently. A major component of the program is medication education and adherence monitoring.
