Program to provide Comprehensive, community-based Care to frail, older adults, who meet nursing home level of Care. Criteria Care is provided both at home and at a dynamic day health center and is coordinated by an interdisciplinary team. All services and products normally covered under medicare and medicaid are provided.
Comprehensive Senior Care Corporation dba Senior health Partners mission is to improve the health and wellbeing of older adults and caregivers in calhoun county and surrounding communities, following the guiding principles that include the belief that older adults should be treated with dignity, respect and have access to an integrated continuum of Care and community resources. Serving approximately 900 seniors and caregivers per month at 25 different locations throughout calhoun county.
