We believe in the importance of educating this vulnerable population about the vital role of home health and personal care services in achieving these goals. For the tax year ending 2022, we have successfully implemented 4 distinct educational programs aimed at fulfilling our mission.1. Home health services 101: in partnership with local healthcare providers and experts, affirmation organized a series of workshops and seminars to educate Senior care residents about the basics of home health services. These sessions covered topics such as medication management, wound care, physical therapy, and the importance of regular health assessments.2. Safety in the home: affirmation conducted interactive safety training sessions to raise awareness about common home hazards and prevention strategies. Residents learned about fall prevention, fire safety, and emergency preparedness to ensure their well-being within their homes.3. Independence enhancement: our organization worked with occupational therapists and professionals to offer seniors practical advice and hands-on training to maintain their independence. This program focused on mobility, adaptive technologies, and daily Living skills.4. Personal care services: affirmation provided information and training on personal care services, including in-home caregivers and support, helping seniors understand the role of personal care aides in assisting with daily activities and improving their quality of life.5. Medication management program: this program is designed to promote seniors' health, safety, and independence by facilitating a comprehensive and structured approach to medication management. The program includes a comprehensive medication reconciliation process involving patients, healthcare providers, and pharmacies; weekly pill box filling and nurse consultation; and medication reminders. This meticulous approach guarantees that seniors' medication regimens are accurately documented and monitored.
