Sevamp encompasses the overall management and supervisory functions of the area agency on aging, as well as the lead role for providing a coordinated comprehensive system for older persons to locate and access needed Services. Senior Services' mission is to provide seniors and their caregivers with access to programs and Services, so they may live their lives with choice and dignity in their communities. Supportive Services funded by title iii-b of the older americans act:provided the following Services: legal Services for 40 individuals for 143 hours; provided transportation to 759 individuals for 30,907 one way trips, information assistance and referrals to 1,956 individuals for 8,205 contacts, provided long term care coordinating and planning activities for 4,043 hours, provided public information and education for 181 events, provided socialization events for 259 individuals for 12,699 hours of activity, and we helped 129 individuals with emergency needs.
Home delivered meals funded by title c-2 of the older americans actprovided 163,719 meals delivered to the home of 1,201 individuals.
Congregate meals c-1provided 61,044 meals to 1,160 indivduals in a social congregate setting.
Caregiver Services - title iiiethe caregiver Program helped seniors and their caregivers receive a variety of Services. The Services include: care coordination for 43 seniors for 194 hours; provided adult day care to 52 seniors for 25,000 hours; provided transportation to 16 individuals for 928 one way trips to day care Services, information assistance and referrals to 680 caregivers for 1,312 contacts: provided 2,324 hours of homemaker Services to 22 caregivers and provided 96 public information and education sessions for 3,201 caregivers.
Health promotion provided the following Services; provided care transition from hospital to home for 192 individuals with 592 contacts; provided options counseling for 341 individuals for 693 hours; provided assistance for 5 individuals concerning elder abuse; made 2,986contacts with individuals concerning their medicare benefit; 185 individuals participated in 1,848 evidenced based wellness classes and provided 31 people with nutrition counseling.
All othersother Program service areas include 50 Senior companions served 88 seniors with 53,457 hours of Services, we provided 32 people with 2,069 hours of in home homemaker servcies; we provided 57 individuals with 7,269 hours of in home personal care
