The mission of Service Coordination of South Central pa, Inc. is to assess the needs of individuals and recommend services, which will enable individuals to live as independently and safely as possible in their own homes. Service Coordination of South Central pa, Inc. will help to coordinate the needs of individuals with all available community resources. Through continual evaluations, we will maximize and manage services to meet participants' current and future needs. Assessing needs, recommending services, & coordinating resources to help people live on their own.

Service coordinators work with participants to identify and coordinate state funded in-home care services. Service coordinators work to obtain services from local resources and link those services to achievemaximum independence for our participants.service coordinators help participants in setting up a plan for care. They also assist the participant in choosing an agency to provide the in-home care services.in-home care services support adults with physical disabilities who need help to perform daily living tasks.
