Resident Care Services: as one of the Texas gulf coast's larger nonprofit medicare/medicaid and cms/hhsc licensed skilled nursing facility and assisted living residence for the aged, Seven Acres provides Care to the fastest-growing demographic in the country: those age 65 and older. A full roster of medical personnel are trained to diagnose and treat conditions that often present differently in older adults; develop Care plans that address special healthcare needs; communicate with families and other caregivers; coordinate Care across the spectrum of Services; and take a patient-centered, holistic approach to maintain each resident's functional status, independence, dignity, and quality of life.our programs offer the following Services:- skilled nursing Care- rehabilitative Care: rehabilitation due to fracture or joint replacement, advanced wound Care, post-operative Care, chronic illness management, non-invasive respiratory Care, parkinson's rehabilitation, stroke recovery therapy, general rehabilitation due to weakness or decline, iv therapy- intermediate long-term Care- intensive long-term Care- hospice Services- assisted livingseven Acres remains a unique institution in greater houston as this is one of only two Jewish homes for the aged in Texas. The campus, comprised of Seven Acres Jewish Senior Care Services and the medallion assisted living, is home to approximately 100 elderly and frail jews and provides a Jewish cultural and religious environment unavailable anywhere else in this area - all while remaining welcoming and inclusive of others. We strengthen the community by providing Care for aged persons following the traditions and practices of the Jewish religion, including prescribed dietary practices. Our residents have contributed to society, supported families, raised children, had careers, served their country, and many have outlived their savings. Seven Acres offers specialty geriatric Care by our physician-led team of nurses, therapists, social workers, and dieticians. Many experience their last days with us in hospice or palliative Care.
