CTG (Closing The Gap Program) The goal of Holden heights family health initiative is to improve their overall health outcomes of 400 Black and Hispanic residents in Holden Heights. The intervention has several components: outreach and screening, health assessment and health education classes and referrals to medical and socioeconomic services. In 2023 the project staff served 420 residents, providing screening for dental and primary care services, enrolled residents in GED and other technical programs. We measure health outcomes. Participants reduce BMI, increased intake of fruits and vegetables and increase exercise activities.
Healthy Start Coalition
The Orange County Fatherhood program services 30 fathers with children 18 years and younger residing in Orange County. The project provides parenting classes and wrap around social services for fathers and families. In 2022, the project staff served over 100 fathers and provided wrap around services to include referrals for medical, employment and housing. A large percentage of the clients became employed with a living wage. Non custodial fathers increase child support payments and became more engaged in the lives of their children
