Community Care program: the Illinois department on aging's subsidized in-home Care program provides a variety of services for older adults who need assistance with activities of daily living on a regular basis. Eligibility for this program is based on the need for Care and financial criteria. A Care coordinator can provide an in-home assessment to determine eligibility for this program. Services provided by the community Care program include: homemakers services, adult day Care, automated medication dispenser, and emergency home response systems.
Adult protective services: Solutions for Care is a state-designated adult protective services (aps) agency through the Illinois department on aging which investigates abuse, neglect, financial exploitation, and self-neglect of individuals ages 18-59 living with a documented disability and anyone age 60 and over. All adult protective services are confidential and free of charge. The Solutions for Care aps team serves berwyn, cicero, and proviso township. Investigating approximately 400 cases annually, with financial exploitation being the most frequent offense.
Aging & disability resource network: Solutions for Care is a state-designated aging & disability resource network (adrn) site that provides up-to-date information and quality referrals to individuals with disabilities ages 18-59 and to older adults and their family members. Adrn also offers the senior health assistance program (shap) which connects our clients with state and federal programs. Adrn conducts services at our main office in north riverside mondays-fridays. The agency also hosts enrollment events at alternate locations throughout the year.
Caregiver resource center: the caregiver resource center (crc) at Solutions for Care is a resource center that serves as a point of entry to a broad range of services and resources for caregivers and grandparents raising grandchildren. The crc's caregiver specialist links older persons, caregivers of older persons, grandparents or other non-parent older adult relatives raising children (who are not more than age 18), and parents or other relatives raising children aged 19-59 with a disability with community members that can provide support in these types of family situations.provides respite, counseling, education and training, support groups for caregivers and grandparents raising grandchildren. Serves only the town of cicero and the city of berwyn.
