Teton Free Clinic

The Clinic was open 2 evening per week for 48 weeks during the year. We hosted monthly diabetes and healthy lifestyle education classes. Attendance in these classes is mandtory for patients who have diabetes. The Clinic is staffed by 3 mds, 4 fnps, 2 pas,and 16 nurses and Medical assistants. We have 12 other volunteers that fill roles in the front office and provide translation services. During the year we provded care through 997 patient visits. The Clinic continues to collaborate with many local health and human service providers and continues to be a pickup site for whole food rescue no cost grocery Program.

The Community foundation of jackson hole awarded a Community impact grant to the Clinic to be used to support the Clinic's pilot Program - Medical management for diabetes in Teton county. The grant covered insulin costs, stipends for Medical care providers directly involved with this Program, administrative costs, and interpretative services. Patients were required to attend classes in lifestyle modification support and social needs assessment provided by st. john's health diabetes education group. Early results show improved patient compliance and improved health.
