Program areas at The Coordinating Center
The Coordinating Center, under contract with the Maryland Department of Health provides case management services for individuals enrolled in Maryland Medicaid's Rare and Expensive Case Management Program, serving on average over 4,000 people living with conditions or diseases that may be considered rare and/or expensive to treat.
Through its Population Health Division, The Coordinating Center works with hospitals, physician practices and managed care organizations to deliver customized care coordination and coaching models for at risk populations.
Community First Programs (CFP), under contract with the Maryland Department of Health, The Coordinating Center provides care coordination services for individuals living with disabilities and/or complex medical and/or behavioral needs, who are enrolled in one of more of Maryland Medicaid's home and community-based programs; serving on average over 3,000 people a year.
Other programs including service coordination and care management services for children with Autism, formerly homeless adults and children and adults with complex medical needs and disabilities. Programs include the Maryland's Home and Community Based Services Waiver for Children with Autism Spectrum Disorder, Montgomery County's Housing Initiative Program, the Model Waiver Program, Maryland Developmental Disabilities Administration's (DDA) Coordination of Community Services Program, VIPhysicians&Kids medical home program, Life Care Planning and Case Management services provided by the Medical Legal Services Division and the CARES Program for PPMCO members.
