Program areas at The Grand View Health Foundation
The medical access program provides assistance to uninsured and underinsured individuals in our community who are experiencing an acute illness. The program is staffed by social workers who assist The patient in finding care for their acute problem and direct them to sources of ongoing medical care. Assistance is provided to access clinics, physicians and medication through established pharmaceutical company programs. Patients are provided with The resources necessary to apply for medical assistance and other appropriate medical insurance programs. Grand View hospital may pay for medications and testing related to acute care illness based on eligibility.
The community Health needs assessment must be performed every three years and gvh will be required to address The needs identified in this report by offering appropriate services, screenings, informational programs and marketing messages to help "lead our community to a healthier future." The amount requested will go toward this year's assessment and The ongoing implementation of The initiatives ide
The Health promotion and wellness staff, in conjunction with other Grand View departments as well as external organizations, coordinates and provides numerous supportive and educational healthrelated programs to The community, most at no cost. Led by Health professionals, these classes feature a variety of topics including cancer, diabetes, heart care, orthopaedics, women's and children's Health, fitness and more. Grand View also offers educational programs addressing The special Health needs of people over The age of 50. Topics include flu, driving safety, fall prevention, and more. In addition, funds will be made available for pediatric patients referred to The Grand new youth program who have a demonstrated need for financial assistance.
Cancer support services and nutrition counseling for oncology patients patients undergoing radiation therapy generally benefit from nutritional counseling to help them deal with The impact of these treatments on their bodies. Medicare does not cover this service and yet it is linked to our cancer accreditation. Funding is being requested to provide these services at a reduced cost or no cost depending on The circumstances of The individual patients. Other cancer support needs such as prosthetics for patients would be covered through this fund.
The healthy beginnings plus program provides care during pregnancy for low-income women. A team of professionals including board certified ob/gyn physicians, specially trained nurses, social workers, dietitians and educators work together to provide care that is individualized to each patient's needs. The goal of The program is to provide ongoing prenatal care to ensure a safe and healthy birth and to minimize The occurrence of complications. Patients served by this program range in age from 13 to 45 years. Also, we are continuing with a program to promote breastfeeding for new mothers consistent with standards established by The joint commission.
The gvh advanced life support service is staffed by highly trained and dedicated state-certified paramedics and emergency medical technicians (emt's) who maintain a variety of certifications essential to providing effective and efficient care during a prehospital emergency situation. Gvh serves The boroughs of perkasie, dublin, and sellersville, as well as, west rockhill, east rockhill, hilltown, milford, and salford townships.
The bucks county Health improvement project ("bchip") is a collaborative effort of bucks county hospitals in conjunction with The bucks county department of Health and bucks county medical society. The project works to improve The overall Health of bucks county residents, by providing a variety of Health related services for low-income county residents. Each year The bucks county Health improvement project requests grants from each of The hospitals in support of The project.
