Servant house: 21 bed transitional housing program for disabled veterans who are experiencing homelessness. Servant house served 51 veterans in 2023-2024. Of The 32 residents that exited The program in 2023-2024, 84% exited for permanent housing and 84% exited with income.
Fast track: a rapid rehousing program combining housing-focused case management and financial assistance to quickly re-house individuals and families experiencing homelessness. During fy 2024 fast track served 231 individuals including 77 children. Of The 87 that exited The program, 97% of The people who exited left for permanent housing and 77% had income.
The disability assistance program assists uninsured and under-insured individuals file for ssi/ssdi benefits. In addition, they utilize a samhsa evidence-based model called soar to assist individuals who are experiencing homelessness to file for ssi/ssdi benefits. In 2023-2024, 235 applications were filed with a 63% approval rate. Also includes veterans stand down program.
Glenwood/ haworth houses are 17 permanent supportive housing apartments for disabled individuals exiting homelessness, with a preference for veterans. Glenwood/ haworth are low rent apartments that come with case management and other supportive services to help The high-need residents maintain their permanent housing. During 2023-2043, 100% of residents maintained their permanent housing. Other programs included are americorps.
