LJ: Palliative Care Work Group Learning Journeys, a project in collaboration with the SF Black, Chinese, and Latinx communities to determine barriers and facilitators for community members to complete advance directives and gain access to palliative care. Thriving in Place is the fiscal sponsor for the Palliative Care Work Group and acted in an advisory role for this project.
PCWG - Palliative Care Work Group - Support community organizations and health systems with training and culturally relevant tools and resources to help their community members participate in advance care planning and access palliative care. PCWG serves individuals who want to learn about or participate in Advance Care Planning and/or access palliative care for themselves, friends, or family members by providing online tools and resources, and connections to community organizations.
Participatory Community Grantmaking Initiative: A project in collaboration with the Palliative Care Work Group and the SF Black and Chinese communities to partner with community agencies and leaders to further develop and implement advance care planning interventions including outreach, workshops and materials. Thriving in Place is the fiscal sponsor for this initiative which brings together a coalition of individuals and organizations. Thriving in Places acted in an advisory role for this project.
