Program areas at Transitions Centers
Day habilitation program - provides year round services to help support individuals focus on independent living and social skills. Other services include nursing services and physical, occupational and speech therapy.
Vocational program - provides individuals with assistance in obtaining competitive paid employment within the community. Additionally, individuals are supported with transportation and social service needs, identified as a hindrance to successful employment.
Transportation program - provides door-to-door transportation for those members who participate in the day habilitation program. The program is funded through human service transportation and is managed by the greater attleboro and taunton regional transit authority (gatra).
Supported employment - assists in placing adults and students with challenges in community employment that best matches the person's interests and abilities by providing a customized plan.
