Program areas at Unitarian Universalist House Out- Reach of Joseph Priestley District
The organization provides an outreach program to assist older adults, ages 60 and older, to remain in their homes, in the neighborhoods of Germantown, Mt. Airy, Chestnut Hill, West Oak Lane, and a section of East Falls. The organization offers home visits to assess individual needs and provide health education and social services to provide individualized care management and counseling, health screening, educational seminars and overall care support with other providers and organizations in the Northwest Philadelphia area. Outreach services are provided at no charge to the individuals.
Health promotion encourages older adults to set and achieve their wellness goals through health education and wellness workshops, individual consultation, and health screenings. Social services provide individualized case management and counseling and access to community resources. In addition, the organization provides supportive services for the residents of the maple village apartments, which is where the organizations office is located. Client assistance fund provides small grants to older adults in need.
