Program areas at UCP
Supported living and building blocks - the supported living program provides support services for 39 individuals in the home/community, which are determined by individual choice and provides the opportunity to live where and with whom they choose for as long as they desire, recognizing that needs and desires may change over time. This program provides training and in-home support to assist individuals in increasing their independence in daily living activities and with completion of household chores. The building blocks program serves 2 families with children with disabilities. The program supports before and after school care.
Support services - ucp connections and ucp mentors is designed to assist adults with developmental disabilities to identify and arrange the support they need to thrive within their community and live the life they choose. Ucp connections and ucp mentors does this by brokering services and resources that assist the people they serve to reach their goals, developing plans and individual budgets using person centered planning techniques. Ucp connections currently serve 362 customers and ucp mentors serves 494 customers.
The supported employment program offers vocational profiles, job development, job training, and ongoing support as necessary to 12 individuals with disabilities.
Information and referral programs - the community development program acts as an information and referral source for more than 800 families with children who have Cerebral Palsy and other disabilities. This program hosts a variety of parent information groups and acts as supportive presence at meetings with professionals that help parents make decisions about a child's life. This program also provides caregiver relief for hourly and overnight respite care for children with disabilities.
