Honoring choices pacific northwest (hcpnw) is a project/program that was developed from the joint collaboration of Washington State Medical Association (wsma) and Washington State hospital Association (wsha), through grants and donations from third parties. Wsma and wsha, through hcpnw, partnered with wsma Foundation to allow the Foundation to collect tax deductible donations for hcpnw and distribute the money for the goals of hcpnw. The program includes an informational website, general education materials, and focused instruction of personnel in hospitals, hospital systems, and other healthcare facilities through the use of certified faculty. The ultimate goal of hcpnw is for everyone in the pacific northwest to receive Care that honors personal values and goals at the end of life.
The Washington serious illness Care coalition is a broad-based cross-section of individuals and organizations across the State interested in improving Care at the end of life. Formed in 1997 their mission is to promote caring conversations about living and dying well within all communities through their annual meeting and support for State polst paradigm program.
The Washington State Medical Foundation for Health Care Improvement creates transformational Health Care change at all levels individual, organizational, and statewide. We believe in a future where physicians are healthy and fulfilled where physicians, the Health Care team, and patients are partners and where all washingtonians experience quality Care that aligns with their needs, goals, and values.
