Program areas at Youth and Families Determined To Succeed
In 2018, Youth Determined To Succeed's (yds) management invested over 58,000 in establishing the organizations fitness & wellness center in north minneapolis To provide access To affordable health, fitness and wellness services and programs To an underserved community that continues To be plagued by high rates of Youth and family obesity and associated diseases such as diabetes. With over 10,000 square feet and wide range of exercise and fitness equipment, yds is able To provide comprehensive and personalized individual and group fitness, nutrition and wellness programs. Our evidence based outcome approach is based on learnings from conducting a three-year pilot, serving over 350 Youth and Families diagnosed as obese To severly obese (70% referred by their physcians), many sufferings from diabetes, hypertension and other related diseases. With the assistance of our collaborative partnership with the university of Minnesota and the mayo clinic, we have begun To develop the framework for conducting a retro study of our pilot and current approach. A working title: "a community-based, patient centered intervention health service delivery model To treat severe To morbidly obese children and Families". This will include evaluating our proof of concept pilot services, health outcomes and patient experience, To further develop yds's collaborative care approach and service model. Ultimately, yds hopes To help Families develop a culture of wellness through restoring their health and establishing a healthy wise lifestyle. Additionally, yds management plans To conduct a comprehensive comparative study in the future.
Community impact - in collaboration with the brooklyn center rotary club, brooklyn park school district and high school, for the 9th year, yds served 80 students through our after-school wellness program that included our ylc leadership program and club fit kids program. In part of a broader community effort To address the growing obesity crisis and health disparities in the community, yds completed a series of 2 community events, To increase awareness of living healthy lifestyles, over 3,000 individuals attended these events.
